Please provide us with the following information to enroll in a course:
Full Name (Exactly as you'd use with your LTC or FID Application including middle initial.)
You will receive a seat confirmation and directions to the class when both this enrollment form and payment is received. To make payment on line via credit card or electronic check transfer, please return to your course page and refer to the Payment Icons on the bottom of the page. Otherwise, you may send a check or money order to the above address made payable to Mike Burchman. Classes are filled on a first come first serve basis.
